Information for New Hires
On behalf of Broken Arrow Public Schools (BAPS), welcome to the District! On your first day of employment*, you will be attending a "Welcome to BAPS" orientation. At this half-day orientation (begins at 8:30 a.m. on either a Monday or Thursday in the Human Resources Department), you will go over many aspects of being an employee with BAPS such as Policies and Procedures, Worker’s Compensation, W-4, Technology, and Benefits eligible to you. We created this page to give everyone the opportunity to get a head start on all the paperwork needed on your first day. Feel free to print (one-sided only please) and complete as much as possible. You may leave any areas you are unsure of blank. We will go over all paperwork during the orientation.
New Hire Forms
- New Hire Paperwork for the Recruitment Team
- Payroll Paperwork
- Benefits Department Forms: to be completed during orientation
- Review the Employee Benefits Option Guide
- American Fidelity Forms: to be completed during orientation
- Sun Life Forms: to be completed during orientation
What to Bring
All new employees will need to bring proper identification for I-9 processing. Visit the official website of the Department of Homeland Security to view acceptable forms of ID. You will need to bring one from List A OR one from List B and List C.
Be sure to bring a voided check or documentation from your bank with routing/account information for direct deposit. BAPS does not issue paper checks. If you do not have a checking account, Payroll will go over an alternative option for payment during orientation.
Bring beneficiary(s) social security number(s) and their date of birth for beneficiary forms you will be required to complete.
Certified staff only: bring Official College Transcripts (including both undergraduate and graduate) and certification.
Bring a smile. We will be taking your picture for your school ID card.
What to Do Afterwards
Since the orientation is held on your first day of employment*, you will need to contact your supervisor for further instructions. Also, both fingerprints and EGID (health, dental, vision) enrollment forms are due back to HR within five days of hire date.
*Note: Applicants hired during the summer for the 2018/2019 school year, will be attending meetings prior to their first day of work in order for staff to begin the new school year with students on the first day of school. You will be notified by the recruitment team if this applies to your position. All benefits will start on the first of the month following your official hire date.
