New Hire Benefit Forms
The following forms may be completed in advance of the new hire orientation. Be mindful these are all legal documents. Write legibly. Use a blue or black ink pen. Try to refrain from marking out or scribbling on the forms. The Benefits team will review the documents to assure all information has been completed correctly.

Oklahoma Teachers’ Retirement System (TRS)
TRS is a state agency that administers retirement assets and provides income security through a monthly retirement benefit payable throughout the lifetime of each member. Employees are vested after contributing to TRS for 7 years. Forms will be available at New Employee Orientation if you are not able to complete in advance. Bring social security numbers, addresses, and date of birth for all beneficiaries.

Cost: 7% of your total compensation annually divided into 24 pay periods.
Certified employees are required to participate; therefore, the district pays the 7%. The following forms are required to be completed:
Support employees working at least 4 hours or more per day have 30 days to make a one-time, irrevocable decision.
Example of how to calculate the cost:
$15,000 annual salary x .07 = $1050 then divide $1050 by 24 pays = $43.75 per pay check
All support employees who do NOT want to be a member must complete the TRS Participation Opt-Out Form.

Summary of Benefits and Coverage (SBC)
The Summary of Benefits and Coverage will summarize important information about any health plan option in a standard format to help you compare across options.
Marketplace Notice under ACA
The Health Insurance Marketplace (also known as the “Marketplace” or “exchange”) provides health plan shopping and enrollment services through websites, call centers and in-person help. If an employee chooses to use the Marketplace, please use the Marketplace Exchange form when submitting an application.
COBRA Continuation Coverage Rights
This notice generally explains COBRA continuation coverage, when it may become available to you and your family, and what you need to do to protect the right to receive it. COBRA continuation coverage is a continuation of coverage when coverage would otherwise end because of a life event.
VOYA, 403(b) Retirement Plans
Enrollment information for retirement savings plan that includes enrollment instructions on how to access your account, track savings and contact information.
- 403(b) and 457’s are supplemental retirement plans with optional enrollment to help employees save for a secure financial future. They are long term investments.

Sick Leave Sharing Bank
All employees working six hours or more must sign up or decline to be a member of the sick bank. More details found in District Policy 5310 and on this webpage.
The Standard Group Life Insurance
All employees working six hours or more per day will complete life insurance forms. <Download Enrollment Form>
The district pays the following amounts for initial life insurance:
- full-time certified and support Staff - $20,000
- directors, principals and assistant principals - $50,000
- assisted superintendent, associate superintendent, deputy superintendent and cabinet - $75,000

OMES/EGID Medical, Dental, Vision, and Life Insurance
All employees working four hours or more per day must complete an Enrollment Form OR Waiver Form.

Coverage is effective the first day of the month following your employment date.
As a new employee, you have 30 days from your employment or eligibility date to enroll in coverage. If you do not enroll within 30 days, you cannot enroll until the next annual Option Period, unless you experience a qualifying event. You have 30 days following your eligibility date to make changes to your original enrollment.
American Fidelity Disability Insurance
All employees working six hours or more per day receive American Fidelity disability coverage.
- The district pays for $7.80 towards the cost of any plan.
- Employee may sign up for additional coverage.
